Overview

The research group is associated with the National Reference Centre for Tropical Pathogens and focuses on the diagnosis, epidemiology, and immunology of emerging zoonotic diseases. We investigate interdisciplinary highly pathogenic bacterial infections (typhus group rickettsiosis, scrub typhus, Q fever, tularemia, and plague), viral encephalitis (zoonotic bornavirus infections), the immunology of arboviral infections, and neglected tissue-invasive parasitic infections (muscular sarcocystosis, visceral pentastomiasis, and larval tapeworm infections).

The group is interested in the development of novel assays for pathogen detection from human tissues and bodily fluids, vectors, and natural hosts. Besides serological and molecular assays, immunohistochemical tests and electron microscopy studies are performed. Moreover, the immunological response of the human host during infection is analyzed from tissue, whole blood and serum. Together with our international partners we conduct field studies in the Democratic Republic of the Congo, Madagascar, Tanzania, and Malaysia.

Epidemiology and immunology of human bornavirus encephalitis

Following the discovery of a novel zoonotic bornavirus (variegated squirrel bornavirus 1, VSBV-1) in a cluster of fatal encephalitis cases among private breeders of exotic squirrels, we conduct molecular, immunological and epidemiological studies on human bornavirus infections. Recently, we could also detect VSBV-1 as cause of an occupation-related fatal limbic encephalitis in a zoo worker who had contact to exotic squirrels. Moreover, the virus of the classical Borna disease (Borna disease virus 1, BoDV-1) could be demonstrated as cause of encephalitis in a cluster of solid-organ transplant recipients, showing the zoonotic potential also of BoDV-1.

Host response during arboviral infection

Several arboviruses cause long-lasting arthralgia after the acute phase of infection. Our analyses demonstrate prolonged elevated proinflammatory cytokine levels in the recovery phase of arthritogenic alphaviral disease. Cytokine level testing in alphavirus in­fections with prolonged arthralgia may aid monitoring pa­tient symptoms. This information is valuable when clinical signs of arthritis, such as joint swelling and redness, are no longer present, and standard serum inflam­matory parameters are within reference ranges.

Clinical Virology

Several individual cases, case series, and cohorts with primarily arboviral infections were analyzed. Passive surveillance techniques enabled us to report and examine the first imported Zika virus infections into Europe, as well as other unusual arbovirus diseases, including O'nyong-nyong and Mayaro virus infections.

Epidemiology and immunology of high-consequence bacterial infections

The research group investigates the epidemiology and immunology (antibody, cytokine and cellular responses) mainly of high-consequence bacterial pathogens that are transmitted by arthropods as well as aerosols, such as Rickettsia typhi and R. prowazekii, Orientia tsutsugamushi, Coxiella burnetii, Francisella tularensis, and Yersinia pestis. Field assays for the rapid molecular detection of these pathogens are being developed. Moreover, the environmental stability of R. typhi and R. prowazekii is analyzed.

Sarcocystosis - Outbreak investigations in Malaysia

An outbreak of invasive human sarcocystosis on Tioman Island, Malaysia was epidemiologically investigated, allowing us to describe clinical and immunological characteristics, as well as treatment options. Symptoms, laboratory changes and kinetics of cytokines and chemokines in the patients demonstrated a biphasic course of disease, from an initial vascular to a later muscular stage. Molecular tests for diagnosis of this food-borne parasitosis due to Sarcocystis nesbittii were developed. Local investigations hint at a fecal contamination from large reptiles as source of the outbreak.

Pentastomiasis - Field studies and analysis of local clusters in rural Africa

The epidemiology, clinical and histological features of this highly unusual food-borne, tissue-invasive parasitic disease was investigated in two areas of local clustering in rural Africa (The Gambia and the Democratic Republic of The Congo), and Malaysia. Clinically, ocular and visceral forms were detected in two prospective field studies in central Africa. Besides immunohistochemical analyses, specific molecular assays were designed for the detection of parasite larvae even in highly necrotic host tissue. At local markets, Armillifer armillatus and Armillifer grandis pentastomids were discovered in a high percentage of large snakes offered for consumption, emphasizing bushmeat as source of infection with exotic pathogens.

Echinococcosis & Cysticercosis - Endemic larval tapeworm diseases

Molecular, serological and immunohistochemical assays were engineered for the detection and follow-up analyses of food-borne tissue-invasive larval tapeworm diseases. With a focus on alveolar echinococcosis due to Echinococcus multilocularis, growth characteristics in human tissue were described in a 3D model. In addition, antibody levels against recombinant antigens (especially Em18) were shown to reflect the clinical stage of the disease and to mirror treatment success or relapse, respectively.

Molecular pathogen detection in archived samples

The identification of (novel) pathogens, especially for the restrospective detection in past outbreaks, is often restrained by the absence of stored serum or cerebrospinal fluid samples. However, from such epidemics, case series, or single cases of unknown etiology, formalin-fixed paraffin-embedded samples sometimes remain in pathology archives for retrospective molecular analysis. The past decade has seen a steady increase in the appilcability of NGS for the detection and characterization of pathogens also from such archived tissue samples. Nucleic acids recovered from these clinical specimens that were not stored specifically for later DNA (and RNA) analyses may provide insights not only in the cause of a past outbreak, but also in pathogen evolution.

Fachgruppen Erregernachweis im Gewebe, Bakterien- und Parasitendiagnostik (NRZ)

Die Fachgruppe des Nationalen Referenzzentrums für tropische Infektionserreger (NRZ) betreut die Antigenherstellung serologischer Tests (IFT, IHA, ELISA, IB) und die Etablierung molekularbiologischer Tests im Bereich Bakteriologie und Parasitologie. Validierungen werden mit entsprechend charakterisierten Referenzmaterialien (Seren bzw. DNA/RNA) durchgeführt.

Desweiteren ist die Fachgruppe für den Bereich Erregernachweis im Gewebe des NRZs verantwortlich. Hierbei werden Viren, Bakterien und Parasiten molekularbiologisch und immunhistochemisch bzw. histologisch im Gewebe detektiert und charakterisiert.
